WOT and VTEC Engage & Disengage

In Closed Loop Settings, there are "WOT MAP Based Engagement" and "WOT TPS Engagement" tables. Does it require both table's inputs to be met before WOT is engaged(RPM/mBar -and- RPM/TPS), or will WOT engage when just one of the table's inputs are met(RPM/mBar -or- RPM/TPS)???
In VTEC Settings, what is the difference between "Lower" and "Upper" engage/disengage? Low Load & High Load? There is also the "VTEC Engagement Window" table, which am I correct to assume that this table is also MAP based? If so, how would that be used if the VTEC Engage/Disengage is set to occur at a specific RPM? Is this a one-or-the-other type situation as well?
Also, in any "MAP Based" table, what would you put in the fields if you do not want MAP Based actions to be made?
